Luxury, Longevity, and the Future of Recovery Medicine

At Carrara Treatment Wellness & Spa, where Dr. Kenneth Spielvogel brings both precision and artistry to care, healing goes beyond the clinical. This month, we’re proud to highlight two patient stories that reflect the thoughtful, results-driven work he performs at Carrara and the transformative potential of our integrative approach.

A New Standard in Recovery: Mr. T’s Six-Month Transformation

When Mr. T arrived at Carrara, he was carrying more than weight. He faced metabolic dysfunction, emotional exhaustion, and a long history of alcohol use disorder. What happened over the next six months is nothing short of extraordinary.

Through a fully customized plan incorporating GLP-1 medications, targeted peptide therapy, structured fitness and nutrition, and evidence-based psychiatric care, Mr. T lost 51 pounds, normalized his labs, and—most importantly—reclaimed his sobriety and his sense of self.

Read his full transformation below.

When Mr. T, a 47-year-old man, walked into our clinic, he was carrying more than just extra weight. With a BMI of 38.6 and a 20-year history of alcohol use disorder (AUD), his physical and emotional health were deteriorating. Alongside obesity, he was also dealing with prediabetes, high blood pressure, unhealthy cholesterol levels, and early signs of fatty liver disease. After a recent ER visit for alcohol-related gastritis, he knew something had to change.

Unlike past attempts that focused on just one piece of the puzzle, this time Mr. T chose Carrara Treatment Wellness and Spa. Carrara provides an unparalleled healing experience combining state-of-the-art therapies with upscale amenities. The facility features private suites with en-suite bathrooms, gourmet meals prepared by top chefs, and a range of holistic treatments, including yoga, meditation, and spa services. With a focus on individualized treatment plans and a compassionate approach, Carrara stands as a sanctuary for those pursuing lasting recovery in a serene and luxurious setting.

We took a comprehensive, whole-person approach—blending medical therapies, targeted supplements, behavioral support, and structured lifestyle change. Six months later, the transformation was not only visible on the outside; but also, deeply rooted in sustainable wellness and long-term sobriety.

The Starting Point: Where He Was

At his first visit, Mr. T reported poor sleep, low energy, high stress, and an ongoing struggle with alcohol relapse despite past rehab and therapy. Blood tests revealed insulin resistance, borderline A1c, and elevated liver enzymes. He also admitted to poor diet habits and a sedentary lifestyle. More than just a medical overhaul, he needed a new framework for how to live.

The Plan: Combining Modern Medicine with Personalized Wellness

Our treatment strategy was built around five core components:

1. GLP-1 Agonist: Zepbound (tirzepatide) To help jumpstart weight loss and improve metabolic health, Mr. T was prescribed Zepbound, a once-weekly injectable that targets appetite and insulin sensitivity. Emerging evidence suggests that GLP-1 medications like this may also reduce alcohol cravings by acting on brain reward pathways—a promising dual benefit in his case.

2. Peptide Therapy He began a peptide protocol that included CJC-1295/Ipamorelin to support natural growth hormone production and BPC-157 to help with gut health and systemic inflammation. These injections, taken five times a week, supported fat loss, muscle recovery, and cellular healing during his lifestyle overhaul.

3. Medications for Alcohol Use Disorder To directly address his drinking, Mr. T started naltrexone, which can reduce cravings and blunt the reward from alcohol. Gabapentin was added for sleep issues and to ease the discomfort that often leads to early relapse.

4. Structured Fitness and Nutrition With help from our health coaching team, he began resistance training three times a week and low-impact cardio five days a week. He followed a Mediterranean-style eating plan, prioritized protein, and adopted time-restricted eating. Calories started around 1800 per day and were adjusted as he progressed.

5. Mental and Emotional Support Weekly CBT-based therapy and participation in recovery meetings helped Mr. T work through his emotional triggers and build new habits. He began journaling, tracking his mood and cravings, and used a mobile app to stay accountable and monitor progress.

The Outcome: A 6-Month Transformation

By month six, Mr. T had lost 51 pounds, dropped his BMI to just over 30, and normalized his blood sugar. His liver enzymes had improved dramatically, and his blood pressure and lipid panels were back within healthy ranges. He reported better sleep, more energy, and a renewed sense of purpose.

Most importantly—he stayed sober. For the first time in two decades, he felt in control, crediting the integrated approach for addressing not just the alcohol, but the whole environment that enabled it.

“It wasn’t just about losing weight,” he said. “It was about becoming the version of me I thought I’d lost a decade ago.”

Why This Matters

This case highlights the power of treating the person, not just the diagnosis. By combining cutting-edge medications like Zepbound, hormonal and peptide optimization, psychological support, and functional fitness and nutrition strategies, we helped a patient make sustainable, life-changing progress on multiple fronts.

In patients with both obesity and addiction—two conditions that are deeply interconnected—this kind of multidisciplinary care may represent a new standard in modern recovery medicine. More research is needed, but the results speak for themselves.

This case also underscores the importance of beginning treatment in a structured, supportive environment like Carrara Treatment Wellness & Spa, where the patient was able to disconnect from daily stressors and fully immerse in healing. The luxury, concierge-style setting provided not only comfort and privacy; but also, immediate access to medical supervision, nutritional guidance, fitness coaching, and evidence-based behavioral therapies. For individuals facing the dual challenges of addiction and metabolic dysfunction, a high-touch inpatient experience can serve as a powerful catalyst—kickstarting motivation, accelerating clinical progress, and laying the foundation for sustainable change once back in daily life.

GLP-1s and Addiction Recovery: A Modern Integrative Model for Healing

A Case in Recovery: Repairing the Body to Heal the Mind

John, a 45-year-old executive, arrived at Carrara exhausted and demoralized after years of alcohol use. His liver enzymes were elevated, his sleep erratic, and his motivation gone. Traditional detox had helped him get sober before—but never well.

At Carrara, we view recovery through a metabolic and neurobiological lens. The first step was stabilization, which involved repairing the liver, calming anxiety, and restoring sleep and the circadian rhythm.

He began taking silymarin (milk thistle) and N-acetylcysteine (NAC) to reduce oxidative liver stress and replenish glutathione; omega-3 fatty acids for neuronal repair and inflammation control; and magnesium glycinate with L-theanine for anxiety and sleep support. Within two weeks, his mood and focus began to lift.

Modern Architecture

Phase One: Stabilization and the Science of Craving

Chronic alcohol exposure rewires dopamine pathways, blunting natural reward and increasing compulsive drive. This is where GLP-1 receptor agonists—initially developed for the treatment of diabetes and obesity—offer a powerful new lever. GLP-1 receptors are active not only in the pancreas but also in the ventral tegmental area and nucleus accumbens, key centers in the brain’s reward circuit. When stimulated, GLP-1 signaling reduces dopamine spikes, lowering the rewarding impact of alcohol, nicotine, and other addictive substances. Recent data are compelling: a 2025 JAMA Psychiatry randomized trial found weekly semaglutide significantly reduced alcohol intake and cravings. Extensive population studies have reported up to 50 % lower relapse rates among GLP-1 users compared with other weight-loss medications. At Carrara, we initiated low-dose semaglutide as part of John’s metabolic recovery—both for weight normalization and to re-tune his dopamine and insulin signaling.

Phase Two: Repair, Regeneration, and Resilience

After three months, John’s energy, sleep, and labs had normalized. This was the window to introduce targeted amino acids and research peptides to enhance neuroplasticity, tissue repair, and stress tolerance.

Investigational Peptides (Research-Use Only)

  • BPC-157 – A synthetic fragment of a gastric peptide with preclinical data showing accelerated healing and reduced inflammation in muscle, liver, and gut.

  • CJC-1295 + Ipamorelin – Stimulates natural growth-hormone release, supporting lean mass, bone density, and recovery; early human data but not FDA-approved.

  • Selank / Semax – Russian neuropeptides shown in small studies to improve focus and reduce anxiety through GABA and BDNF modulation.

While only GLP-1 analogs are FDA-approved, these research peptides offer promising biologic tools when used under medical supervision. Carrara integrates them thoughtfully—never as shortcuts, but as adjuncts to behavioral, nutritional, and physiologic therapy.

Modern Architecture

Phase Three: Lifestyle as Medicine

Once stable, John transitioned into physiologic restoration—harnessing environmental stress to strengthen resilience.

  • Infrared and Finnish Sauna: 3–4 sessions per week stimulate heat-shock proteins, reduce systemic inflammation, and improve endothelial function. Long-term studies link regular sauna use with lower cardiovascular mortality and enhanced mood.

  • Cold Plunge: Short bouts of immersion trigger catecholamine release, boost dopamine, and enhance vagal tone—supporting calm alertness.

  • Breathwork & Meditation: Controlled 5–6 breaths per minute breathing synchronizes heart-rate variability, improving stress recovery and emotional regulation.

  • Guided Exercise: Progressive resistance and low-intensity cardio rebuild mitochondrial capacity, improve insulin sensitivity, and restore physical confidence.

  •

Sobriety became, for John, a foundation for performance—not merely abstinence.

Ethical and Regulatory Perspective

  • GLP-1 agonists are FDA-approved for diabetes and obesity, but off-label for addiction.

  • Other peptides are not FDA-approved and are used only under research or wellness frameworks with informed consent, verified sourcing, and ongoing monitoring.

  • All supplement protocols are lab-guided, emphasizing data over trends.

Reimagining Recovery

Addiction is not merely a failure of willpower; it is a metabolic and neurochemical disruption. Healing requires more than abstinence—it demands cellular renewal, nervous-system recalibration, and metabolic balance.

Carrara Treatment Wellness & Spa’s model blends medicine and restoration:
GLP-1 therapy to rebalance the brain, nutraceuticals and peptides to heal the body, and mind-body practices to strengthen the spirit. Recovery becomes not an ending, but a beginning—a journey from depletion to vitality.
